Travel insurance for US residents who are already abroad

The mainstream US travel insurance market is built entirely around pre-departure purchase, because its core product is trip cancellation, and there is nothing left to cancel once you have gone. Ask Allianz or AXA to cover you mid-trip and the answer is no. The medical specialists are a different story.

US residents face one additional wrinkle that catches people out: some international products that happily sell to the rest of the world exclude Americans specifically. SafetyWing's Complete plan cannot be sold to US residents, and Heymondo's already-abroad option is not available to them either. The plans that do work are travel medical policies rather than trip protection.

If you are staying out long term

IMG's Patriot line is renewable up to 36 months and WorldTrips Atlas runs to 364 days, which makes them the usual answer for Americans living outside the country without becoming formal expats. Note that both step benefits down sharply with age, and neither covers you inside the United States once you return home. Genki accepts any residence including US, with US stays limited to the first seven days of each visit unless you buy the upgrade.

Questions people actually ask

Their products are trip protection plans centred on cancellation and interruption, and the cancellation benefit only makes sense before departure. Their published FAQ requires purchase before you leave. It is a product design limit rather than a judgement about you.
Usually barely or not at all. Most domestic plans provide limited or no coverage outside the US, and even those that cover emergencies will not pay for medical evacuation, which is the genuinely expensive part. Check your plan's foreign coverage before assuming, then insure the gap.
No. US waivers are time-linked to your first trip payment, typically 14 to 15 days, so the window has closed by the time you are abroad. Travel medical plans handle pre-existing conditions differently: several cover acute onset of a pre-existing condition up to an age limit, which is not the same as covering the condition itself.
